Information Pack, Costs and Referral Form
If you would like to make a referral to the centre, please complete the standard referral form, and send along with other relevent information for our consideration.
A copy of the form is available below, or it is available by post from us. Please feel free to photocopy the referral form as necessary. We are able to accept people from throughout the UK and also international clients.
We are able to accept self referral or referral by a relative or solicitor for those clients whose participation in the programme is self funded or funded through a compensation claim. For those clients for whom NHS funding is to be sought, referral should come from a General Practitioner or Consultant. On receipt of the referral form, we are happy to approach your local health authority to seek permission to proceed with an appointment. However it is often helpful to discuss this with your GP or local Commissioner first. The latest version of our information pack is also attached below.
