Welcome to the Oliver Zangwill Centre website, the centre has now closed following a public consultation to gather feedback and hear views on the proposal to cease provision of neuro-psychological rehabilitation.

We would like to thank everyone who has worked or volunteered at the centre and wish former clients well in their future.

The centre’s email account and answerphone are no longer monitored. 

If you need clinical support or to access neuro-rehabilitation services please contact your GP. 

You have a right to request copies of your medical records. This is known as a ‘subject access request’. We will provide you with copies of your records, with any information which you do not have a right of access to removed (redacted). This will be done in line with the exemptions to subject access which are set out in the Data Protection Act 2018. You can find out what these are in our Subject Rights Request Policy.

To make your request you can send an email to ccs.accesstoinfo@nhs.net. We will need to see proof of your identity, a copy of your passport, birth certificate or driver's licence and proof of your address, a utility bill, council tax bill or similar, sent to your address in the last 6 months. Your NHS number and that you received support from the Oliver Zangwill Centre.

If you are asking for access to someone else’s records, we will need to see proof that you have a right to make that request on their behalf e.g. a copy of a child’s birth certificate or a letter showing power of attorney has been granted or their signed consent, where they are able to give this.
